Insurance Plans We Accept
Maple Life Solutions currently accepts the following commercial insurance plans:
- Aetna
- Blue Cross and Blue Shield
- HealthChoice
- Optum
- Oscar Health
- UnitedHealthcare
- UMR
Insurance coverage varies by plan, provider, and service. Our team can help verify your benefits before services begin. We also encourage you to contact your insurance plan directly with questions about deductibles, copays, coinsurance, or specific coverage.
Please note: Maple Life Solutions does not bill insurance for ADSAC assessments, ADSAC classes, or group services. These services are self-pay.
SoonerCare / Medicaid
Maple Life Solutions does not currently accept SoonerCare/Medicaid.
Self-Pay
Self-pay is available for people who do not have insurance, who prefer not to use insurance, or who are seeking a service that is not billed to insurance. Contact us for current self-pay rates for the service you are interested in.
Sliding-Scale Options
Sliding-scale options may be available for eligible services based on availability and individual circumstances. Not every service or situation qualifies, so please ask whether a sliding-scale option applies to the service you need.
Payment Methods
Cash, debit cards, and credit cards are accepted. Payment is generally expected at the time of service unless other arrangements have been made in advance.
ADSAC Assessments, Classes & Group Services
ADSAC assessments, ADSAC classes, and group services are self-pay and are not billed to insurance. Current ADSAC assessment fees are set by Oklahoma law and are listed on our ADSAC Assessment page. Current ADSAC class fees and course information are available on our ADSAC Classes page. Contact us for current rates for group services.
